B. Get Your Louisiana Tax ID Number and Or Other State Required Documentation
Have your Federal Tax ID Number (EIN) handy; most states require it on their state tax ID number forms and other forms that you may need to complete after you incorporate a business in Louisiana.
1. File Application for Louisiana Tax Number
You must file the Application for Louisiana Tax Number once your entity is formed and you have a Federal Tax ID Number. The form is at http://www.rev.state.la.us/forms/taxforms/16019(7_02)F.pdf

2. Filing Your Articles with the Recorder of Mortgages of the County (for Corporations Only)
Within 30 days after filing your Articles of Incorporation (not LLC Articles of Organization) you will need to file a certified copy of the Articles and the Initial Report, and a copy of the Certificate of Incorporation with the recorder of mortgages in the parish where the corporation’s registered office is located. (You can also file the other original Articles of Incorporation and Initial Report you executed.)
The Secretary of State will send you a certified copy of the Articles of Incorporation and the Certificate of Incorporation a week or so after you file.
Call the Recorder of Mortgages in the parish in which your registered office is located and ask for filing information.

LLCs DO NOT have to file with the Recorder of Mortgages.

4. Annual Reports
LLCs and corporations must file an annual report on or before the anniversary date of their formation. The filing fee is $25. The annual report form will be mailed to you by the Secretary of State about 60 days prior to its due date. Failure to file will result in your entity losing its good standing to transact business in Louisiana.
