C. Get Your Massachusetts Tax ID Number and Or Other State Required Documentation

1. Massachusetts Trustee Tax Form, TA-1
To register your new entity with the Massachusetts Department of Revenue, you must file online at http://www.massdor.com/options/TA1/ta1_home.htm , also known as Form TA-1. The filing of this form will establish your tax obligations with the Department of Revenue.
You will need to file this form after you obtain your Federal ID Number (EIN).

3. Annual Reports
Corporations must file an annual report with the Secretary of the Commonwealth within 2.5 months after the end of the corporation’s fiscal year; in most cases your fiscal year ends on December 31, so you would need to file the annual report by March 15. The report restates much of the information contained in the Articles. The fee is $125; an additional $25 is charged if you file late. For more information, log on to http://www.sec.state.ma.us/ Choose the Corporation Division. The Pre-filled Annual Report can be obtained there.
LLCs must also file an annual report on or before the anniversary date of the filing of the original Certificate of Organization. The report must contain all of the information included in the original Certificate of Organization. The filing fee is $500. For more information, log on to http://www.sec.state.ma.us/ . Choose the Corporation Division. The Pre-filled Annual Report can be obtained there.
